An unnamed protagonist happens to find a diary in a hospital one day. The diary belongs to his classmate, a girl named Sakura Yamauchi, who is revealed to be suffering from a terminal illness in her pancreas, and who only has a few months left to live. Sakura explains that the protagonist is the only person apart from her family that knows about her condition. The protagonist promises to keep Sakura's secret. Despite their completely opposite personalities, the protagonist decides to be together with Sakura during her last few months.

I Want to Eat Your Pancreas explores an unnamed male high school student coming to befriend a girl his age named Sakura Yamauchi, who is dealing with a terminal illness involving her pancreas. The film is pretty much a by-the-numbers drama involving the male lead learning to bond with Sakura and attempting a heavy-handed moral with our nameless lead learning to befriend others and not be so introverted. The major problem with this film is that the bond developing with our two leads doesn't feel too genuine due to how bland both their characters are. Our male lead feels largely like an audience surrogate of sorts for the film's plot as he lacks much in the way of personality and while Sakura's a lively lead, she doesn't have much dimension that lets her be more than the "tragic sick girl" character type played out from similar titles with this story mold. The side characters in the film are also largely inconsequential to the exploration of our nameless male and Sakura's developing relationship as well. Overall, this is a pretty bland anime drama as it doesn't really offer much to explore beyond its story formula, as it largely felt like it was going through the motions with it.
