Most times this will refer to coarse characters who behave or talk in a lewd or offensive manner, but it can also include situations or dialogue seemly intent on trying to offend the viewer with its socially inappropriate and outrageous content. The heavy metal series Detroit Metal City (TV) easily qualifies for this keyword.
